December 16th 1961 Born in Fukuoka, Blood type B
1980 Graduated Fukuoka Prefectural Moji High School
1986 Graduated Kyushu Dentist University (34th)
1986 Worked in Okamoto Dentist Clinic (Fukuoka)
February 1989 Established Tsukamoto Dental/Pedodontia Clinic
April 1997 Moved and constructed new Tsukamoto Dental/Pedodontia Clinicand continue to operate clinic to this day
